your interval is 1 minute. you set "upalertafter 4m", which means that
a failure which lasts 1 interval will have the upalert squelched.

       upalertafter timeval
              The upalertafter parameter is specified as a string
              that  follows  the syntax of the interval parameter
              ("30s", "1m", etc.), and controls the triggering of
              an upalert.  If a service comes back up after being
              down for a time greater than or equal to the  value
              of this option, an upalert will be called. Use this
              option to prevent upalerts to be called because  of
              "blips" (brief outages).
