Bit of an update on this. I've managed to recreate the problem outside of MonoTouch ie. Just in Xcode, so it doesn't look like it's an issue with between the two.
Odd thing is that even some downloaded samples XIB files don't automatically open the correct file in the Assistant Editor. I've checked all the filenames *.h/*.m match up.
