Think I've managed to fix this - bit of a PITA to do but in the end had to
re-install Xcode 4.2 and then MonoTouch.  No idea why this was necessary
because Xcode / MonoTouch were installed in the correct order on a fresh Mac
Lion installation.

I can get now get a simple MonoDevelop's temporary Xcode project to index
correctly and hence the "Assistant Editor" now works
