Unfortunately, we have no way to diagnose this problem right now. The NSS library nearly always returns a meaningful error code that would tell us what the problem is. Unfortunately, for many of NSS's error codes, PSM doesn't display useful messages about them, but instead calls them "unknown reasons". If PSM simply printed out the NSS error code number in decimal, that would tell us much more than we know now. There is a PSM bug filed about this issue.
