GSM operators in Europe reap huge profits from keeping the SMS prices 
high. Messages from operators that offer free sending (like the one in 
South Africa) get blocked fairly quickly.

As for for-fee services:

Managing SMS fees is not mozilla.org's core activity.

If there is a standard interface to for-fee SMS gateways (I doubt it) 
then it might make sense to file an RFE about implementing support for 
such interface (with each user paying the free him/herself).
