This is a stab in the dark since I haven't look at this in detail, 
however I have encountered the same issue.  It seems to be an issue when 
running Netscape 6.2x and Mozilla on the same machine.  I would assume 
they are using 2 slightly different cache methods.

When I encountered this - I notice right away it was when I ran both 
together.  I tested this by closing both, clearing my Mozilla cach and 
all went back to normal.  I then reopened Netscape 6.2x, browsed around 
a bit and the problem immediately re-appeard.

Try it out - I'm pretty sure this must be your issue.
