Is support for doing various things with different MIME types (passing
off to a helper app, showing with a plugin, displaying inline, saving to
disk) an extension?

If not, then why should support for doing various things with different
protocols be an extension?

Their UI would be one and the same -- the `Applications' category of
prefs. Protocols would (probably) be listed in the same listbox as MIME types.
