Thanks!  I have added this to my collection - I have 4 such samples now,
where the problems are caused by Lame switching (or in some
cases not switching) to mid/side stereo.  I think Lame needs a better
switching critereon.  I have been meaning to try out the algorithm in:

Johnston and Ferreira, Sum-Difference Stereo Transform Coding, Proc. IEEE ICASSP 
(1992) p 569-571. 

where they switch to mid/side stereo not based on FFT energies, but
instead by comparing masking thresholds between the L & R channels.
I'd like to try this because this is the reference the MPEG AAC encoder
papers cite for their mid/side stereo algorithm.
