For VBR 0 lame is still using a loop to choose the bitrate for each frame. Although it's fine for choosing bitrate when a distortion is allowed, I think it's perhaps not the best thing when over=0. In this case, the psy model gives exactly the allowable noise, so it should be possible to encoding directly from this, and looking only after what was the bitrate needed and adding the header.
