I made some hack another routine, so I made full package...

removed hang up mono mode, include Leonid's table fix of loop.c,
Robert's l3psy fix.
but I can't find which makes memory error in high bitrate mode, so this
bug is still remained.
P.S.
Because of changing l3psy, lame-1021 is little bit slower than 1017-4,
but mdct routine is faster than 1017-4.
