My understanding is that San Francisco had significant problems implementing IRV because of voting machine and paper audit trail issues.

In the meantime, those interested can receive a DVD copy of the session from
the media office at the city.
Attending the study session were council members Benson, Johnson Lee,
Lilligren, Ostrow, Samuels, Zimmerman and council member Zerby's aide. Also
attending were County Commissioner Gail Dorfman, Charter Commission member
Todd Ferrara, staff people from the Hennepin County Elections Office, two
city council candidates and some interested residents.
The session focused on why IRV, how it works, where it's being used, and
issues regarding implementation at the local level. Tony Solgard, President
of FairVote Minnesota, Susanne Griffin, Director of Elections for the City
of Minneapolis, and I presented and there was discussion throughout.
The council members were engaged and had good questions and comments about
the value, implications and mechanics of IRV as well as some good insight
regarding the impact of IRV for candidates and voters in local elections.
There are some important issues to further explore regarding the
practicality of IRV for local elections, most importantly required changes
in voting equipment, voter education and the potential use of ranked-choice
voting for all local boards (not only city council and mayor) elected in
odd-year municipal elections.
As a follow-up to the city council study session, a similar presentation
will be made to the Charter Commission at their meeting next Wednesday, May
4th at 4:00 p.m. in the city council chambers.
