Have you tried running the "Find Duplicates" query wizard?  This should show
you any duplicate records in your table.

I am not sure how to eliminate the duplicate item, but if you do not 
want to manually delete them create a first query that groups 
everything on the table and then do your calculations to figure out 
the commission.
