If you are saying that new manual installs are failing to notify the management server to be placed into pending - then the answer will be either someone changed a global setting for rejecting or approving agents, or there is a connectivity issue and the answer will be found on the agent OperationsManager event logs, and/or the MS event logs.

In brief: All manual installs within the same domain/different domains/workgroups are all fine during the install on various versions of Windows Servers, verify port connections just fine and none shown anywhere in SQL table/console/powershell cmdlet, this is a new symptom as it worked before. The only change to the environment was applying the latest available SQL MPs update a couple weeks back. SCOM2012 R2 - for various reasons such as Workgroup/Non trusted domains, etc... that we have a few manual scom agents install ... whether new manual installs or existing ones no longer worked and need a reinstall, they are all manually install/reinstall just fine on the client sides but they "no longer" shown up in Pending Management queue for approval.
