I run it with the default options, and just indexing the CM database. Checking fragmentation before and after, it made a huge difference.
Also, to clarify on Daniel’s comment, SQL 2012 SP2 was not a magic bullet. We are still working with Microsoft Support on post-R2 SQL performance issues, and looking to bump up SQL Server resources as well.
