JoyNet is no longer a proposal, it's a standard!
By the way, I'm working on a game for it, but I won't say much more until I

Does this mean that external memorymappers are the only hardware items that
will benefit from the modification?
And according to Alwin, not even all external memorymappers use the clock
on the cartridge port.

This is a good idea, and makes your 7MHz MSX more compatible with the
standard as well.
But I have some doubts about the usefulness of the 7MHz clock on the other
pin.

How do existing cartridges handle this? Are the reserved signals not
connected or are they connected to the ground or ...?

I think that most future expansions won't need the clock signal anyway,
because they have their own clock (like the MoonSound).
