The Opie plugin looks at the contents of the fields that are synced to detect changes. However, if you start with a contact in both databases and then sync you will end up with duplicates. Once you have everything syncing with no duplicates then nothing should duplicate if you stick with the same sync pair, unless you modify data in both databases simultaneously.
