http://muscleplugins.alioth.debian.org/ has the most recent common source code 
of the v1 applet. It does work on JCOP 41 cards at least and there is a build 
file specific to JCOP.  I don't believe it includes support for 4096 bit RSA, 
but it should be fairly easy to add. If you order enough cards from a vendor, 
you can usually arrange for the applet to be placed on-card during 
pre-personalization - the amount and cost differ from each vendor.
