Maybe its not fixed.

I just had this sequence:
01000130 eventhandler.c:303:EHStatusHandlerThread() Error
communicating to: Aladdin eToken PRO USB 72K Java [Main Interface] 00
00
00400258 ccid_usb.c:649:WriteUSB() write failed (2/13): -4 No such device
00000033 ifdwrapper.c:341:IFDStatusICC() Card not transacted: 617
00000051 utils.c:68:SendHotplugSignal() Send hotplug signal to pcscd (pid=9790)
01000180 eventhandler.c:303:EHStatusHandlerThread() Error

00400237 ccid_usb.c:649:WriteUSB() write failed (2/13): -4 No such device
00000031 ifdwrapper.c:341:IFDStatusICC() Card not transacted: 617

which is pattern "B".

No this would not help, if I understand it correctly. Even if it is
delivered over ContextThread
the problem would be the same.
