I'll answer as best I can; I am not the final authority on policies at
Mutopia.

The feeling a get from past discussions is that in general rewriting a
piece is frowned upon -- although I can totally understand the intention.
Maybe there could be two submissions for the same piece?  I've seen that
before.

I would frown upon changing the pitch of a note that "sounds wrong" to you
unless you have some evidence to back it up.  I would want the piece
exactly how Joplin wrote it.  A footnote would be acceptable, I think.

The footers of the pieces submitted to Mutopia are created "automatically,"
meaning you don't have to change them.

There is no agreed upon file naming convention.  There is some directory
naming conventions, but even these are sometimes not followed.  My personal
rule is to keep the structure of the original piece as closely as
possible.  If you've rewitten the piece, maybe submitting it with a
different name would be a good thing.
