I don't normally used Thunderbird.  Searching the web a found a few
references to Thunderbird using Thread-Index fields to thread messages,
but nothing about setting the field.  According to a quick test, it does
not appear to set Thread-Index.

I did find reference to Evolution using Thread-Index and inserting
References headers based on Thread-Index [1].  However, another quick
test of Evolution indicates that it doesn't preserve Thread-Index.

But since mutt sucks less that either of those, it can do better!
