I think mutt should be enhanced to make that kind of thing as easy as possible - also for the benefit of many other applications. It seems there is actually not much missing or it might even be implemented somehow but but I have now idea where and how. Thinking on something like my_hdr `fancy-script %HThread-Index` and it would be awesome if other modifiers such as %e - mutt variable were also available.
