The second patch does another thing as well; introduces to muttrc.5
the definition and use of the .EX/.EE macro pair for wrapping code
examples that should be set in fixed-with type. It's not in Linux
man(7), yet, but is often found on manual pages from older Unisx
systems (especially DEC Ultrix).
This change makes it easier to automatically translate the page
to clean DocBook-XML.
