 Leaving expectations aside, I think the current semantics are fine. The
 hook patterns should *literally* match the folder URL, ignoring the
 existence of {{{imap_user}}}.

 I have tried this literal configuration ({{{mutt -F muttrc}}}):
 {{{
 set imap_user=peter
 set my_pw="`...`"
 set folder=imaps://imap.lekensteyn.nl/
 set imap_check_subscribed
 account-hook . 'unset imap_pass'
 account-hook imaps://imap.lekensteyn.nl/ 'set imap_pass=$my_pw'
 }}}

 When I change folders (c), the account-hook is still triggered with 1.5.23
 plus the two attached patches. In a debugger, the `Incoming` variable is
 however still NULL. Hmm.

 Do you really see different behavior?
