Can I ask what Mutt you're using? Here I'm using the Mutt 1.5.20 that ships
with Debian Squeeze. With that, I'm able to speak to Exchange via IMAP and
SMTP directly, without requiring fetchmail (ew) or msmtp (nice, but no longer
necessary.)

The config's relatively straightforward... Replace the things in brackets
with your information, and of course don't include the brackets.

Nah, this is just the right place.

Two further notes... If you set biff notifications and have a typo in a
mailbox name, you'll likely see a very misleading SSL error. Double-check
your mailbox directives. Also, note that you can pretty easily integrate
addressbook functionality via an LDAP command. If you get all this working
and you're interested in that, write back to the list and I'll describe it
for you.
