After spending some time researching some of the alternatives, there are a 
few things that I think set MLO apart from the others:

MLO seems to be the only option in its class that offers a full-featured 
alternative to a hosted/cloud-sync service.  Many of the other GTD-Style 
applications appear to limit their application features if the cloud option 
is not used.  For example, an android app might be able to sync a limited 
set of fields to a google account, but in order to sync all of the fields, 
one would need to use the developer's cloud service.  In contrast, MLO 
gives you the option to sync all of your mobile information directly to the 
desktop application using wifi sync.

Being able to bypass the cloud sync option is important for several 
reasons.  For many people, cost might be the first thing that comes to 
mind.  Some might also have concerns about data privacy/security.  I think 
it's also worth pointing out that owning your own data is important.  At 
any given point, a given online service may close up shop for one reason or 
another which means that all of your data would no longer be accessible to 
you.
