about the In-Reply-To field, so how do I get Thunderbird to use it?

Try just clicking the "Reply" button on the button bar, not the link
in the message header. Thunderbird should handle the reply correctly,
it's pretty good at that.
The problem is when I want to reply to a message that I have not
received in my Inbox. When browsing the list at
http://mythtv.org/pipermail/mythtv-users/2005-March/thread.html the only
way to reply to a message is to click on the sender's name. When I
click on the link, it opens up a Compose message window in Thunderbird
with the subject field automatically filled in ...
You're not really replying to the message in that case. The online
archive doesn't work that way for replies. If you want your reply to be
threaded properly, subscribe to the mailing list and reply from your
inbox using your mail client.
