Make sure that you select the option to use the PVR-350 video out in mythtv's setup screens. (Sorry, I forget which one off the top of my head.)

The setting is mythfrontend:
Setup -> TV Settings -> Playback

Go to the screen titled: "Hardware Decoding Settings"

Select/enable: "Use the PVR-350's TV out/MPEG Decoder"
