I personally might word this portion:

To help defer the increase, for every client you send me..

or

As a long standing client, I would like to offer you a bonus for every referral...

either way, when the new client comes in and rebooks (that was my rule.. when they showed up for the second appointment, the referer got the reward),, I would write on the back of a card..
New Client - Mary
Bonus Client - Susie
        $ off, Free whatever
I might offer to allow them to gather the cards and use several at once.. but maybe not., they might hit you on a bad week and you were expecting that income to pay the electric bill..
So in that case I might add:
        Bonus rewards may not be combined

OH so now thats a lot of writtting :) I would popover to Vistaprint and get a batch of cards made up so you just have to fill in names..

now for the cards - you can either give them to the new client to give to the bonus recipient or paperclip to your appointment book, or make some kind of "tickler file" system.. I don't know how I would do that if not using a paper appointment book.. I used to just move forward to Susie's appointment and make a note there she got a bonus.. Well I suppose you could do the same for the electronic systems.. there should be a NOTES section there???
