I built a flyback supply for my oscilloscopes, using a National Simple Switcher part. You might be able to do the same. The transformer requires winding, but it's only one long winding.

You'd probably need to use an EE40 or so core size. (I think; it's been 10 years.)

The advantage of the flyback supply is that it is easy to get it to behave, and it doesn't require a high voltage power MOSFET.
