They have 5 connections:
Sleeve (ground)
Tip
Tip switch
Ring
Ring switch
They are inherently 'balanced' wireable but the way yours has been done is unbalanced. The switch contacts are used for 'normalling' so for an insert return for example your tip and ring switch contacts come FROM the preceding output stage (usually with a jacj wired as a 'listen' and the tip and ting contacts would go into the next stage or piece of gear. Examine a jack carefully and you will see that the tip and ting tags are slightly thicker than the switch contact tags. As you say R is also grounded in your setup, which is to be expected since it is unbalanced. You need to unlink this so that it can become the 'cold' phase of your balanced cabling.

PS I haven't really examined your picture and assume you have 5 tags. The other possibility is that you only have 3 tags (no switch contacts). In this case you can't do 'normalling' but only 'listens' or in / outs.
