I do this by modifying the relevant portion of the .nasl script (usually desc[]). The 
changes will show up in the report regardless of the format (html, text, etc.) I also 
modify the severity level of the report by changing the report type: security_hole, 
security_warning, security_note.

Be sure to save copies of the modified files, though, since nessus-update-plugins will 
overwrite any of your changes... something like:

nessus-update-plugins
cp mynasl/*.nasl /usr/local/lib/nessus/plugins/
kill -HUP `cat /usr/local/var/nessus/nessusd.pid`
