Would they contemplate developing an application
without first putting together some form of design?
Or starting a new project without any plans?

Because that's effectively what the MIB is - it's the
design document for the management problem
that you're trying to address.  (Or part of it, at least)

As Magnus says - this MIB doesn't need to be made
publically available.   It can quite reasonably be kept
private.
    But it would be seriously short-sighted to attempt
to extend the agent without first having the MIB structure
that you're intending to implement.
