A couple of things that I noticed from your output file:

So this *does* seem to be firing the notification event - at least once.

The other thing that may be relevant:

This shouldn't be an Existence entry - it ought to be generating a
Boolean monitor.
It's a bit difficult to tell, but is there a space between '!=' and '0' or not?
There should be three parameters:
     extResult     !=       0
and merging these will probably confuse the monitor parser.

Try making sure that these are separate, and run the agent again.
If things still don't work, can you please post the full results of
walking the complete dismanEventMIB subtree.
