Maybe we should disable threaded MPM in a 2.0.22 tarball by default?

I think we all agree it needs major work.  We don't need more feedback
to tell us that the threaded MPM is a mess.

I'm kind of thinking that it would be easier to toss the current
threaded MPM code and come up with a brand new one that addresses all
of the issues.  (The threaded MPM code is spaghetti for those who
haven't dared to look at it recently.)  -- justin
