Must admit that I have not been paying that much attention to this
thread.

Angus can you list the sources you have set-up, giving the full path for
your update_source and any other ftp sources you may have set-up.

For 1 of the rpms that you show listed as both available and as
installed run: rpm -q "name_of_pkg".
Does the version and release match that of the rpm shown as available
and from what source is it said to be available (i.e. Disk2,
update_source, ftp etc.)

I do not think that you have a db problem but but you may very well have
a screwed/screwy urpmi.cfg
