What you have to do is a slow, tedious process, but you have to
systematically try different combinations of Monitor, displayb settings
and video card until you find the correct combo.

OR

Setup as your hardware dictates as far as your card and display
requirments go and choose the lowest color depth available and go from
there. That was what finally worked on our CPA's machine. As soon as I
lowered the color depth it worked fine.
