That card is known generically as a Mach64, I think. You may have some luck basing your XF86Config on the file found on this page:
http://goingware.com/laptop/linux/config/index.html You will likely need to change the resolution and timing settings for your monitor, in my case this is fixed at 1024 by 768 for my laptop.
