OSM is a 'crowd-sourcing' project.

The users of the map are encouraged to contribute to it by adding to the 
data.

A popular way is to collect the routes & POI's using a GPS unit & 
uploading the data it produces.

Alternatively, Walking Papers, where you can print off an area & 
annotate it with details of your walk.

I also find a camera & pencil/paper to be invaluable as an aide-mémoire..
