The dust cover's were gone & it had a lot of play. The foot peg & bracket came with it. ($12 on e-bay!) I got the tiny punch marks lined up. They were only off about an 1/8 of an inch. The pivot is lubed. It shifts smoother now. I don't know if both of the peg bracket's were bent, but the shift-lever tie-rod sure is close to the oil cap. Did you clean and grease the pivot for the shifter when you had it apart ? detail, details...
