It looks like you're using decoding when viewing an email but not when
replying.  This should give some clues; see repl(1), and mh-format(5).

    $ cat 1
    Subject: =?utf-8?Q?We=20are=20down=20to=20the=20wire.?=
    From: Team PrezCon <[email protected]>
    Reply-To: =?utf-8?Q?Team=20PrezCon?= <[email protected]>
    $
    $ mhshow +. 1
    from:    Team PrezCon <[email protected]>
    subject: We are down to the wire.

    $
    $ cat fmt
    %<{from}To: %(decode {from})\n%>\
    %<{subject}Subject: Re: %(decode {subject})\n%>\

    FIXME
    $
    $ repl -form fmt -editor cat 1
