You would if you were paying for the infrastructure. Speed is an important factor in user experience and SEO elevation. Also, if you use PageSpeed, YSlow or Chrome's tools you'll end up writing xHTML and CSS much better - most sites can be rewritten with 50% or less of the markup. The obvious common design factors such as SQL/Redis or whatever will have performance as a major consideration when you're designing around them - that doesn't mean you should ignore delivering pages of smaller size too. I once worked on a Rails project that was so naively written that it was initially taking five and a half minutes to deliver a fairly simple page (it had, IIRC, 170+ gems) and we're not talking about database time either ....
One of the things refreshing about Node after 6+ years with Rails is that you can build things the way you need - it's not forcing you in any particular direction (which wasn't really suitable for some projects.)
