FYI, we have been notified by another client today that they are unable to
replicate databases off of Cloudant because of this bug. They have attachments,
but no attachment is larger than 2MB. It may be that multiple attachments are
being batched up and still running into the `max_http_request_size` being
small, but it's unclear.
