   This did not work for me, but I can reproduce with a standalone james jpa 
app.

   A change of netty 4 is that a write() call does not flush automatically:

   But I've added several flush()-calls to many context/channel/outputStreams 
and this doesn't work either. If I analyse the network traffic 
(`-Djavax.net.debug=all`), I can see many parts of the email, but I'm missing 
the closing `tag +OK FETCH completed`.
   
   Where is this response created? I'm trying to debug it, but I didn't find 
out so far.
