Review Comment:
Disposing this per-call transport cannot cancel an in-flight send: `Dispose`
cancels `_lifecycleCancellation`, but `FlushAsync` passes only the caller token
to `SendAsync` (and likewise omits the lifecycle token during response-stream
acquisition). If the handler is blocked and the caller does not cancel,
`Close`/`Dispose` can leave the RPC pending indefinitely while the shared
parent remains open. Create a linked operation token under the lifecycle lock
and use it for both the send and stream acquisition.
