I understand what a region is in emacs land, but I don't understand how it translates to mail land. Notmuch is not to my knowledge capable of tagging a sub-region of a message, so in that sense it might not work. If you goal is to tag only a subset of messages in a thread, then regions don't seem like the most natural way to do that, since regions are non-disjoint.
