I am going to agree with the password problem, If a users changes there password without logging off all the machines this will happen. To help determine when the use is getting locked from use this article q189541.
This will show you how to enable netlogon logging. Once enabled you can view the log at c:\winnt\debug\netlogon.log, this will show every authentication that is attempted. Simply open the log file and do a find for the username until you see an entry followed by the lockout code which I think is 000234 This will also tell you what workstation they are logging on from and you can reboot that workstation. I deal with quite a bit and 99% of the time using this method I fix the issue in under 15 minutes.
