Thank you Mojca, it seems to make sense all of a sudden: I think those
two accents are simply a mistake, and we should replace the <upsilon, accent>
sequence by the appropriate Unicode character.  That's what Alexej Kryukov
has done in his antomega package (see lines 1271 and 1816 of TeX Live's
texmf-dist/tex/lambda/antomega/ograhyph4.tex).  Thomas?
