protocol.list file examples

I want to look for ALL ports, tcp, udp from 1-65535.  Does anyone have a
protocols.list file I could use?

I tried adding services like:

SSH=ssh
Smile=5000-6000

Etc.  The machine rejects them.  What is the proper format for the
protocols.list file?  Or do I just put stuff in the /etc/services file?
