Small orgs tend not to have the enormous amount of risk management and process (ITIL or otherwise) that large organisations have. Large orgs also have their problems, but something as routine as employee termination does tend to have a standard procedure that does get followed (well, maybe they can still access the carpark). We have a small mountain of documentation to cover thousands of procedures - but there are probably still gaps.
