I know when we used Trend Scan Mail for Exchange, it had the option for some
of the actions to put mail into a spam folder. Maybe you have a rule setup
in that 3rd party tool to move certain types of spam to the junk Mail
folder, and it's not Outlook at all.

OR If it was a few people, possibly that there is a corrupted rule. Running
a /cleanrules will delete all rules in the active Outlook profile. I know I
like to setup rules to move messages to this folder because it has the
"empty Junk mail folder" on the right click. Great for short-term, not sure
if I want it server alert notifications.
